The TV star posted the shocking allegations on her Facebook today along with a picture which appeared to show her with a bald patch on her head.
She wrote a message which alleged Crawford, a scaffolder from Durham, had hurt her.
Chantelle, 26, claimed: "Clark Crawford thanks for the baldy patch the cracked ribs , knocked 2 veneers out ect !!! You're a keeper ? ?…"
There is currently no evidence that Chantelle has reported her allegations to the police.
The Sun Online has attempted to get in touch with Crawford for a response to the TV star's claims, which are unsubstantiated, but have been unable to contact him.
It is not clear whether the couple are still together, but hours after Chantelle posted the claims, Crawford was still listed as "in a relationship" on his Facebook page.
Chantelle's relationship status has not been made public.
The reality regular is believed to have left the country as she posted an update on her page yesterday which revealed she was travelling to Gran Canaria.
She later added a saucy snap of herself posing completely naked with her "gay bestie".
Chantelle joined Geordie Shore at the beginning of 2016 as part of the new additions to series 12.
She confirmed she was leaving the show in June after only one season after having a meltdown during filming in Greece,
The Sun Online revealed she stormed off set during the shoot in Kavos and returned to Newcastle to reunite with her on/off boyfriend Tommy Sayers.
The couple split within two months.
The star later came forward and accused MTV bosses of orchestrating a relationship between her and Lothario Gaz Beadle, and that the show itself was faked.
Chantelle was also linked to fellow reality star Jemma Lucy before striking up a romance with Crawford.
