The coaches, Vickie and Allan at Planet Penguin Football Foundation, founded the Brentford Football Club for players with Down's Syndrome, while also running clubs for men and women over 50.
Vickie and Allan thought they were appearing on the show to discuss their football teams, but Alex made a confession which shocked them.
The duo had been nominated by their clubs and friends to be recognised by the BBC One show and appear on the One Big Thank You segment.
Alex was stood on a green football pitch outside of the studio and said to the duo: "I'm with Vickie and Allan who have set up three different community teams at their club, including a team for those with Down's Syndrome and an over-50s men's mental health team and a brand new women's walking team."
She then looked at the duo and said: "I'm going to come clean. So you know we did a rehearsal earlier and I asked you those questions? It's not why you're here.
"Let me tell you – those people who play in your teams got in contact with us and they told us how much you two have changed their lives."
Alex then showed them a series of video messages from those who voted for them as she declared: "This is your one big thank you."
After they watched the clip, their football team along with three former Lionesses, Alex Scott, Rachel Yankey and Ellen White showered them with hugs.
Alex Scott told the pair they were both "amazing humans" for bringing people together.
After the segment ended an emotional Alex Jones praised how well it went, before adding: "I need a tissue!" as the show came to a close.
The One Show continues weekdays on BBC One at 7pm.
